Meanwhile, new version of Demul was released, although I don't think it will improve anything regarding emulation of Shenmue in any way, here is changelog:
Demul v0.7 alpha 160814

- fixed hang in KalistiOS based games with DC keyboard controller
- fixed hang in some WinCE games if VMU present
- better Dev.Box emulation
- patches for Cycraft InitialD and ClubKart (boots, but not yet playable)
- better aspect correction and filtering for Cave CV1000
- InitialD, ClubKart and Derby Owners magnetic card readers emulated (press PUSH6 key when game ask insert/take card)
- fixed Marvel vs Capcom 2 character pictures during intro
- ClubKart2003 input fixes
- WWF Royal Rumble 4 players inputs
- NAOMI DIMM Board emulated
- added recently dumped NAOMI and Atomiswave sets
- romset in sync with MAME
- bunch of minor things not worth mention

All I ask of them is to resolve the Vsync audio issue that's all. I have it pretty much down, except for tearing when running fullscreen, which seemingly vsync is the only resolve. But if I enable that, then the audio goes out the window.

Also, I thought you couldn't play Windows CE DC games? Can it be done? Im dying to play Tomb Raider: Last Revelation.

Windows CE games are playable with the latest build, so you can enjoy Tomb Raider TLR.

- fix current LBA after Mil-CD security check (Pier Solar playable again)
- ADPCM decoder tweaks, fixes Shenmue I 'muted' sound issues(Fuku-san's piggy bank,"Yokosuka Blues" tape, etc..), and probably other games that had similar problems with sound before.

How does the installation of the new build work? Do I need to remove my bios files and roms, delete the old install and replace it with the new one? Or is there a previous version detection?

Get the latest build from OP (now includes new audio tweaks so it should run Shenmue games better too), copy your roms folder from your old build into the new one, reconfigure the new build, save your new settings and you are good to go. You can still use the old build if you like as well.

Unfortunately, it didn't fix the audio issue during Shenhua's monologue (Shenmue II prologue).

This version has voices files from the Xbox version of Shenmue II injected into the Dreamcast game since English-dubbing wasn't officially implemented. This process resulted a few audio-sync glitches here and there throughout the game including the opening scene. This has nothing to do with the emulator since it perfectly runs the official PAL and Japanese versions of Shenmue II without this very audio-sync glitch ...

Quick question - is the 160818 build linked in here more advanced than the 180816 build on the Demul website? I figured higher number is better, but some of the old build numbers start with a 2.

160818 is the latest build(more advanced). Actually i'm not sure if theres is latest build on main site right now it might not been updated because.. and I quote
