I seem to remember it giving pretty clear instructions on what to do at any given time. It may have an open world, but it's more Zelda than Elder Scrolls; you generally have to go where the game tells you to, rather than wandering off. Go somewhere too soon, and nothing will happen there.
I wouldn't say it's over-hyped at all; the hype comes from the variety of gameplay situations it offers, the unique world and story it presents, and the inventive way it uses the new game+ feature. It requires that you get pretty far into it to appreciate these things, though. A couple hours just won't show it off. And, for the most part, it's better if these things are left to surprise you as well.
I was a bit put off by the apparent "hype" it got as some kind of cult gem a ways back myself, but when I finally caved and started seriously playing it, I realized the praise was all well deserved.
Except for the fishing. Unskippable (contrary to what Dorian kept saying), and incredibly user un-friendly.
